Lodge a Claim

Step 1

Contact the claims department at Australian Income Protection on 1300 559 362 (landline only) or (02) 8252 7900. Alternatively you may download a claim form from your appropriate policy holder below.

Step 2

There are 3 sections, Section A (Claimant to complete), Section B (Doctor to complete) and Section C (Employer to complete).

Step 3

Ensure all questions on the claim form are completed in full, if you have any questions please don’t hesitate to contact our claims department.

Step 4

Post original claim form and additional documentation to:
